    1974 ORDINANCE NO. 4

    AN ORDINANCE KNOWN AS MEDICAL CERTIFICATE AND IMPOSING A FEE OF TWO PESOS.

     

    Section 1

    Every laborer, employee, helper, housemaid and houseboy to all kinds of establishments shall be required to secure a medical certificate from the Rural Health Physician, and in his/her absence from any private medical practitioner and to pay the amount of two pesos (P2.00) to the office of the Municipal Treasurer of his/her duly authorized representative.


    Section 2

    This medical certificate is renewable on or before January 20 of each year.


    Section 3

    Any violation of the provision of this ordinance shall be punished by an imprisonment of five days of a fine off fifteen pesos, or both fine and imprisonment at the discretion of the court.


    Section 4

    Any ordinance or parts thereof which are inconsistent to the provisions of this ordinance are hereby repealed.


    Section 5

    This ordinance shall take effect on January 1, 1974.
